The NI Primary Care Network has produced an 'explainer' infographic summarising the new ‘Neighbourhood Model of Health and Wellbeing’ for Northern Ireland.
The new model, which was published on 31 March 2026, is a core element of the Reset Plan for health and social care services in Northern Ireland. It is intended to mark the beginning of a major shift in how care is delivered to meet rising demand.
Implementation of the new model begins in April 2026 - 17 Integrated Neighbourhood Teams will be established to serve communities across Northern Ireland, working within GP Federation areas. These teams will plan and deliver care together, building strong links with the smaller neighbourhoods they support.
