Health and care systems are facing growing demand, widening inequalities and increasing pressure on public services. The answer is not simply to do more of the same, but to accelerate a shift to a new model of care – one that is proactive, preventative and rooted in communities. This conference will explore how a Neighbourhood Health Service is the vehicle for this transformation – moving beyond organisational boundaries to create healthier populations, stronger communities and more sustainable public services.
The conference will examine what those conditions look like in practice. Drawing on national and international learning, we will explore how clear ambition, sustained investment and a relentless focus on relationships can enable services, communities and local partners to work differently together.
Most importantly, this conference challenge leaders to think beyond short-term pressures and organisational performance measures. Achieving a population health model requires alignment across funding, workforce, leadership and accountability, alongside a collective commitment to long-term transformation.
